In-Home Supportive Services (IHSS) is California's Medi-Cal program that pays for in-home care so eligible seniors and people with disabilities can remain safely at home instead of moving to a facility. How IHSS works

  1. Apply through your county. Each of California's 58 counties runs its own IHSS office; the application starts there (Hailo's full guide lists every county office).
  2. A county social worker assesses needs. They visit, evaluate what help is needed — bathing, dressing, meal preparation, housekeeping, paramedical tasks — and authorize a number of monthly care hours.
  3. An enrolled provider delivers the care. The recipient chooses their provider, who completes county enrollment (orientation, background check) and is paid by the program for the authorized hours.
